Looks good.
Just one comment. The buttons Submit, Preview, Cancel and Add File are unreadable on my PC until you point the mouse at them.

Sharon, I am getting no picture at all; just text and boxes. Is this correct? I don't have a problem with tjat but your original post suggested there would be some sort of graphic. Just want to make sure everything is okay with what I am seeing.
